Substitute Notice – Potential Disclosure of Patient Information
St. Vincent Evansville Hospital
The privacy and security of patient information is of utmost importance to St. Vincent Evansville Hospital ("Hospital"), and it has implemented significant security measures to protect such information. Regrettably, despite the Hospital’s efforts to safeguard patient information, an inadvertent disclosure by an employee has affected Hospital patients.
On December 20, 2017, we were notified that patient information included on a 2015 Peripheral Vascular Lab patient tracking log had been inadvertently disclosed to a third-party organization by an employee.
The patient information included patient name, encounter date, encounter number, medical record number and abbreviation for the type of procedure performed. The information did not include patient social security numbers. Upon discovering the incident, the Hospital has confirmed all documentation was properly destroyed and no information was retained.
Based on the investigation, we have no reason to believe that patient information has been used inappropriately. However, we encourage you to regularly review use and access of your personal information (e.g. credit card statements, bank statements, etc.) in order to quickly detect any misuse. The Hospital regrets this unfortunate incident and we are taking the necessary and appropriate steps to prevent this incident from occurring in the future. Specifically, we are continually evaluating and modifying our practices to enhance the security and privacy of all confidential information. In addition, as part of the Compliance Program, the Hospital requires all employees to complete compliance and Privacy training annually.
