Community Health Needs Assessment
To increase the overall health and wellness of our community and to comply with new regulations in the Affordable Care Act, a local collaboration formed amongst St. Vincent Health, Deaconess Health System, ECHO Community Health Care, The United Way of Southern Indiana, and the Welborn Baptist Foundation. This collaborative joined together to assess and identify the primary health needs of people living in our communities.
The Community Health Needs Assessment (CHNA) is an assessment of the communities' health status, barriers to care, and other demographic and social issues affecting our community. The assessment will help focus resources and drive change to support healthcare prevention.
St. Vincent Health is pleased to present the 2013 (Tax Year) Vanderburgh County and Warrick County Community Health Needs Assessment. These reports provide an overview of the health needs and priorities in our community for both St. Vincent Medical Center and St. Vincent Warrick Hospital:

The goals of these assessments are to 1) provide an understanding of the health needs in our community; 2) guide our collaborative planning efforts for the next 3 years; and 3) to serve as the basis of an implementation strategy that addresses prioritized needs.

This plan states how the collaborative will partner with the community and the plan to address the priority health issues determined by the community health needs assessment. We strongly encourage all community organizations and individuals to think how they can impact the community health issues and be a collaborative partner in efforts taking place in the community to address health.
